I have a daily routine, shower with Hibaclenz, use my topical meds and keep up with Cosentyx which it’s 1 shot for 4 weeks. The Cosentyx works really good. Soon I’ll be able to stop everything but the shot. Adding: I also found new panties that don’t make you sweat. They are called Knotty Knickers. They are wonderful. Kinda spendy, but well worth it.

I have found that keeping my groin dry is the most helpful. If I’ve been out in the heat my underwear get replaced as soon as I get home. As I write this I’m even thinking of keeping spares just to cut down on time in sweaty underwear. Or if I’m at home going commando has been super helpful in the healing process.

When dealing with HS in the heat , reduce the activity whether it's warm or hot.

Other thoughts. Maybe a little personal but I’m seeing we’re all dealing with it. I found some underwear that I love. They are called Fit For Me by Fruit of the Loom. They are briefs, of course, but there’s two things about them that’s really helpful for me. The material has got tiny little holes in it for breathability and the leg holes are the stretchiest of any underwear I’ve found. That way they don’t pull on my skin but move with it. I even buy a size up if I’m close (on the size chart) to give a looser fit around the legs. I tried several different underwear shorts and even though they say they have a gusset (for liners and pads) it’s not the kind that is at all useful. Those kinds are so stretchy everywhere that nothing stays put. (I was trying to avoid having elastic run straight through my groin.) For me, it’s best I only shave half way up my thighs or maybe even just above the knee. Shaving any higher is a guarantee of developing a lesion. Lastly, if you can, I find it helpful to forgo the underwear overnight. Good luck, Everyone!

I have started using salycilic acid soray and it has almost dried everything up in just one week. A miracle for me. Nothing else has worked. I figured this out on my own.
