So I was on Humira and it helped my psoriasis get better but made my HS worse. So they changed me to Cosentyx injections but told me I couldn’t start them till I didn’t have a flare up going on. The problem is I never don’t have a flare up! I am in constant pain and always have at least fifteen spots on my body. They are starting to show up all over and they are so painful! It seems like everyday I get a new one and they get bad quickly and most spots keep filling up and they hurt so bad. They… read more

It sounds like you're going through a really tough time with your HS flare-ups. Here are some key points from my knowledge base that might help:

- Cosentyx (Secukinumab): This is an immunomodulator approved by the FDA to treat moderate to severe HS. It works by inhibiting interleukin 17A, a chemical involved in HS inflammation. It's typically administered every four weeks, or every two weeks if the response is inadequate.

- Starting Cosentyx During a Flare-Up: Your doctor advised against starting Cosentyx during a flare-up. This is likely due to the need for your body to be in a more stable condition to better assess the medication's effectiveness and manage any potential side effects.

- Managing Flare-Ups: While waiting to start Cosentyx, consider these tips:
- Avoid Scrubbing or Picking: Scrubbing, exfoliating, or picking at lesions can lead to infection, bleeding, and scarring, delaying healing.
- Professional Drainage: If a lesion needs to be drained, have it done by a dermatologist in a sterile environment.
- Support Networks: Connect with others who understand HS on platforms like myHSteam, where members share advice and support.

- Biologics and HS: Some people have reported that biologics like Humira worsened their HS, leading to a switch in medication. It's important to monitor your symptoms and communicate with your healthcare provider.

Remember, it's crucial to follow your doctor's advice and keep them informed about your symptoms. They can provide the best guidance tailored to your specific situation.
