Don’t use razors. Use wet/dry battery shaver and soak head of it in alcohol afterward. I stay shaved with that to avoid having so much bacteria. Hair breeds more germs. But I don’t remove hair in crease of groin because hair there reduces friction. Imho I think antibacterial make it worse by destroying natural skin flora. I use cereve soap for normal skin. I use a different wash cloths for arm pits, another for groin, another for feet to avoid cross contamination. Sounds like you also have hormone issues. Are you near menopause? Go low saturated fat low carb Mediterranean diet. Less than 14 grams saturated fat a day, 10 grams better. Absolutely no alcohol, None!. No hydrogenated fat like in coffee creamerNo sugar,Eating olive oil and fish helps me. I wear men’s cotton loose boxers. I use silicone bandage tape and just toilet paper for bandages, the silicone tape is great because no irritation and can unstick it and restick it to check bumps. Walmart equate brand has it. Cheap and doesn’t pull at scabs. Wear loose clothes and sleep without underwear. No tights, no panty hose, The friction causes buttock outbreaks. Keep blood sugar tightly controlled. Absolutely no alcohol. Fat free yogurt and low fat or fat free milk kefir very good to consume. Use calorie counter app that checks your vitamin and minerals in diet. I use cronometer.com. Vitamin and mineral deficiencies also cause outbreaks. Take half a 50 mg zinc pill during flares. Use hot compresses to drain bumps. I use unscented cereve soap for normal skin. Don’t scrub hard with wash cloths. Use just hands in delicate skin areas with the soap. Use hot wet compresses and hot tub soaks to drain pus and ichthamol black salve. If won’t drain go to ER to cut them open to drain. Dermatologist approach made me worse. I avoid antibacterials like the plague. I’ll use hibeclense for a few days to remove biofilm if a real bad patch of boils but that’s it. Then I go back to cereve and hot tub soaks 3x a day. Keep blood sugar tightly controlled. Any time my blood sugar is high I have outbreaks. Use a cheap Walmart blood test meter, super cheap and test first thing when wake up. Test after carb laden foods to see which ones affect you the worst. If you have blood sugar issues and you clean up fatty liver with low carb Mediterranean diet your skin will start looking great,

It sounds like you've been managing your condition diligently, and your dermatologist has ruled out many other possibilities. If it's not HS, other conditions that mimic its symptoms include acne, boils, cysts, or folliculitis. However, your history and symptoms align closely with mild HS, especially since it's localized and flares intermittently.

For ruptures, consider using non-stick wound dressings or hydrocolloid bandages, which can absorb drainage and protect the area. Loose, breathable clothing can also help reduce pressure and irritation. When you see your dermatologist, you might ask about topical or oral treatments for flares, even if prescriptions for HS aren't yet warranted. Keep advocating for yourself—you're doing great!
